In the not so distant past when your kilt was delivered by one's kilt-maker there was always a pair,sometimes two pairs, of tartan boxer type shorts included, without being specifically ordered. I am not aware that this nice thought is included by any kilt-maker these days.

It was not so much a "nice thought" as that these undertrews were simply regarded as a standard part of Highland Dress (civilian and officer).
From about the middle 30s, they were usually separately itemised. From about the middle 80s they ceased to be available.
