You're in the S.F. Bay Area. There's a good leather shop called Image Leather on Market Street in the Castro district that will make a belt for you according to your specifications.
They have a lot of other leather items besides belts. I don't know what those things are for.

Lots of the local Street and Arts Fairs have leather worker booths that will make stuff for you on the spot. That's how I got my indestructible day-wear sporran straps. They usually cost a bit more, but if the item lasts you fifteen years, who cares?

Re-enactment sutlers are great sources for various leather items, including wide belts. Here's a neat one with an early (18th c.) style buckle for only $24.00:
