I think this is it...
The 3" piece labeled Inner fold of first pleat, goes back under the first pleat all three inches. Then the Under Apron Pleat simply folds from there to as far under the "over apron" as it will go, and folds back, leaving simply a very large inner pleat between the first pleat and the over apron.
Is this a correct interpretation of the instructions ?

I have no idea where I "want" to trim it. Can I just leave it 16" as specified in the diagram ? Should I trim it at less than this ?

It depends on how large you are and what your requirements for modesty are. You can't go wrong leaving it at 16"-this provides very good coverage in the wind.
