Try a waistcoat with lots of pockets, perhaps you can do without the sporran!

I have had spinal fusion at C5,6,7 at L4,5,&6 they need to also come out. However what the Doctor tried first was he inserted small probes into the nerve bundles. Then with some low volt electricity he burn the nerve bundles out. Now after 15 + years of pain into my leg I have been pain free for 5 years. However sometimes my belt and sporran does give me some localized pain so when I'm know I am going to be out for a long time I wear braces (suspenders) they help a lot.

A sporran is not necessary! Some of our members, the Wizard of BC, seldom wears one. While I like the look of one and have made many of them if you try to carry everything you believe you need in them they can get heavy and cause back problems.

Pretty much just wallet and flask of whisky is all I carry - maybe I should get a smaller flask

How many pockets come on a standard waist coat, just the two I'd think? I will keep this in mind, could be an option for days a sporran would make a long day longer.

The flask is just fine, it's the wallet that's too big.

And here I was looking for some way for my back problems to be an excuse to wear a kilt!

I have a ruptured disc that gave me extreme pain down my leg at one time. If I do too much lifting or bending it flairs up again. I avoided surgery and now hike (kilted). There's my excuse! I'm now looking for ways to strengthen muscles at my stomach and lower back.
