I have had remarks about my boots, but rarely in place of a kilt comment.
At least one friend of mine (a mental health professional) has noted that she doesn't have the patience to deal with strangers requesting her time in public like that. I suspect some of my other friends have been asked questions about me more than once. SWMBO certainly has.
It's just a by-product of my lifestyle/wardrobe choice.

I was kilted this weekend as a friend and I attended a craft fair. Passing one of the booths, a woman was staring at my kilt (or so I thought). I looked at her and smiled and she said, "I like those little flags you have!" She was referring to my flashes. I said, "Oh, thanks. They keep my socks up!"
All in all, an odd little exchange.

I often get asked where I bought it, because they have a son or brother who would like one.
But I also get the shirt or shoes compliment, it's just a comfort level issue for the person.

Yes, Jock, we chaps are "for real". On the west bank of the Pond it's much more acceptable to comment on another person's clothes or hairstyle or shoes or car than I think it is on your side, and some people can't remember the word "kilt", and they know not to say "skirt", so when they see someone dressed "out of the norm", they feel a need to comment. To not look foolish, they think of the first word that comes to mind, which might be shoes or hat or the little flags on your socks. Or, they'll say, "That's a pretty plaid. Is it your family's?", which leads to a discussion on tartan, clans, and all sorts of Scottish stuff. A lot of the time, the person will mention his/her neighbor or cousin or milkman who went to Scotland, and then we "have something in common".

Well Jock, I think we're just used to it. It's like asking a guy from the UK or Australia how he can stand driving on the left side of the road. He'll shrug and ask how one can tolerate being on the right.

The grass is greener on the other side of the fence...and it's usually greenest right above the septic tank.

There is the well-known comeback to the under-the-kilt question to consider:
"Shoes and socks!"
Perhaps the young lady was familar with that and was making a joke about the joke.... BTW is that her in the photo and did you get her name or phone or other contact info? 
Second thought, good as the first: were your shoes polished to a high sheen? Could she see a reflection?
