The Trickers' from Sierra Trading were UK sized and didn't fit, though they were as lovely as you'd ever hope to see. Return for refund (less shipping) was simple enough, but it didn't get me much closer to footwear Nirvana.

Allen Edmonds have very good quality, but I managed to get three pairs of the same shoes (San Marco) in three sizes (8,8.5,9) that all seem to fit me about the same. Either something was amiss in the size markings or I really need a pair of 8.375s. I found a couple of other pairs of AEs that were lovely, but either a different last or another size marking error caused me to give them away. My brother is grateful and beautifully shod these days.

They refund VAT for appropriate customers and they will accept returns. But shipping is 25 GBP one way. I bought a plain pair of what they call Country Tramper shoes. The scar is healing nicely from where they took my kidney and the guy did pay me in cash.

Yes, that's pretty much the only way you're getting custom shoes for less than the price stated in your OP
