Dress shirts, polos, t-shirts inside. Sweaters, sweatshirts, hoodies over the kilt.
Aloha shirts over the kilt worn open with the t-shirt tucked in.
Culture bending for sure...

I ALWAYS have at LEAST a t-shirt under the kilt. Depending on my level of dress and the temp, often I have on more. Unless the upper layer is only just a wee bit longer than the pants waist (not hang below my rear), I tuck it in. On my hockey jersey, for example, the lower part in lucked, while the rest hangs. To help visualize:
 
The navy/red/yellow stripe is just hidden from view as it hangs inside out (like the reverse of a pleat).
I do the same with sweaters and sweatshirts that "hang" to low.

It is general considered "proper" by most to not have anything hang much below fell in the rear.
Jackets that are either "waist" length or cut off look better than long ones:

 
NOTE: ALL of the kilts I wear now are "traditional" high waisted. For "hip huggers," I would probably tuck in more things fully or not at all.

I'm one of the "shirts in; sweaters out" crowd. My rule of thumb: if it has a waist band (ie. sweaters, hoodies) it stays out, if not; it's tucked in.

Same here. I think tucked in for a shirt & T-shirt just makes a more gentlemanly apperance.
