Four Sons Brewpub in Titusville, PA had a fully kilted staff when I visited a few years ago. Great food and great beer but I understand they closed up the end of last year hopefully to re-open with new owners.

Funny, each time we've passed by and the one time we ate there I didn't see any girls that looked like that! They have seemed to appreciate my kilt though. In downtown Cincinnati we have a Celtic themed restaurant called Nicholson's. The manager/owner regularly wears a Kilt, as do most of the wait staff. In fact if a customer shows up kilted, they usually get a free drink from the bar.

I the Detroit area we have The Full Kilt! Only one of the owners wears a kilt, he's a piper too.

Here in St Louis we have a place called the Scottish Arms, Great food, waiters are kilted and waitresses wear mini kilt

jdb beat me to it. You usually have to ask for your free pint.
On the other hand, this place is nearly always empty except right before a show at the Aronoff Center. however, the Claddah Irish Pub (chain) no kilts in sight and always packed. Some of this is location though. Nicholson's owners also run "The Pub" which is a Brittish themed place, and a few of the bartenders wear kilts some of the time.

Like Rocky in Philly, Vancouver and the surrounding areas have dozens of Celtic themed pubs. I have at least 4 (two within walking distance) near by me and I am in one little section of the burbs. Downtown Vancouver has at least 8-10 that I can think of. Most don't have staff in kilts though. Doolin's has, but I can't think of any of the others doing that.

There's an ostensibly "Irish pub" in Flagstaff, but I can't think of any reason it's called that. The pub food is distinctly American, and at night, they play hip-hop music. No kilts to be seen.

Our own "Jack of the Wood" pub has excellent pub food, a good Scotch selection, and one or two of the Sunday night barkeeps wear black kilts. One of them makes his own.
