Mmm... bung from a wine / beer cask?
It's a dual-purpose combination belaying pin and cider tap.

In a democracy it's your vote that counts; in feudalism, it's your Count that votes.
How about a peace pipe? Is there a larger flic?
A left handed smoke shifter? My first thought was a tap, but I think that's wrong.

I would guess a neck and tuning peg for a one-stringed instrument.
So...what is it?
OK, OK it is: a valve tip of an irrigator, an old one with bakelite parts, meant to be sterilised, no one use item as they are nowadays. No, it was not in use, I got it still in its original packing and it fits nicely as an occupational pin, I am a geriatric nurse.... And as you can see it looks good!
